DOSE PROGRAM

Anyone interested in enrolling in the DOSE Program must do so by coming to our main office at 3937 Western Blvd to schedule and Intake Assessment. Intake Assessments cannot be scheduled or completed by phone. Deposit of $25 due at scheduling of intake assessment, the rest of the payment (another $25) is due at the actual appointment. Allow 30 - 45 minutes for the completion of the Pre-intake packet.

Orientation/Group 1: $25
Groups 2-4: $25 each
Groups 5-26: $25 each
